    Opportunistic P2P Communications in Delay-Tolerant Rural Scenarios

    Opportunistic networking represents a promising paradigm for support of communications, specifically in infrastructureless scenarios such as remote areas communications. In principle in opportunistic environments, we would like to make available all the applications thought for traditional wired and wireless networks like file-sharing and content distribution. In this paper, we present a delay-tolerant scenario for file sharing applications in rural areas, where an opportunistic approach is exploited. In order to support communications, we compare two peer-to-peer (P2P) schemes initially conceived for wireless networks and prove their applicability and usefulness to a DTN scenario, where replication of resources can be used to improve the lookup performance and the network can be occasionally connected by means of a data mule. Simulation results show the suitability of the schemes and allow to derive interesting design guidelines on the convenience and applicability of such approaches

    An innovative AAL system based on neural networks and IoT-aware technologies to improve the quality of life in elderly people

    Nowadays more and more elderly people need support in daily activities. This is due to the increase of cognitive diseases and other conditions which lead the elderly to not being self-sufficient. Considering this, providing an Ambient Assisted Living system could improve significantly people life quality and could support caregivers' tasks. The combination of Ambient Assisted Living systems and information and communication technologies achieve this purpose perfectly. They exploit internet of things and artificial intelligence paradigms to make daily challenges easier for people with neurodegenerative diseases. This work melds technologies mentioned above providing a smart system for elderly to manage goods and fill in shopping lists. It was possible using software, hardware, and cloud systems combined with a neural network aimed to recognise products. The proposed system has been validated both from a functional point of view through a proof-of-concept and quantitatively by a performance analysis of its components

    Virtualization of set-top-box devices in next generation SDN-NFV networks: the INPUT project perspective

    Due to the emergence of Software Defined Networking (SDN) and Network Functions Virtualization (NFV) paradigms, coupled with a hyper-connectivity communication paradigm, the \u201csoftwarisation\u201d of the Internet infrastructure and of its network management framework is gaining increasing popularity. This is the target of the INPUT platform, a novel infrastructure and paradigm supporting Future Internet personal cloud services in a more scalable and sustainable way, and with innovative addedvalue capabilities. The INPUT technologies enable next-generation cloud applications to go beyond classical service models, and even replace physical Smart Devices, usually placed in users\u2019 homes (e.g., set-top boxes), with their virtual images, providing them to users \u201cas a Service\u201d. In this paper we present the Virtual set-top box from both architectural and functional points of view, demonstrating the feasibility of the softwarized SDN/NFV paradigm joined with the fog-computing approach to support personal cloud services

    Juvenile hemochromatosis associated with heterozygosity for novel hemojuvelin mutations and with unknown cofactors

    Background & Aims. Juvenile hemochromatosis (JH) is a rare autosomal recessive disorder characterized by severe early-onset iron overload, caused by mutations in hemojuvelin (HJV), hepcidin (HAMP), or a combination of genes regulating iron metabolism. Here we describe two JH cases associated with simple heterozygosity for novel HJV mutations and unknown genetic factors. Case 1: A 12 year-old male from Central Italy with beta-thalassemia trait, increased aminotransferases, ferritin 9035 ng/ml and transferrin saturation 84%, massive hepatocellular siderosis and hepatic bridging fibrosis. Case 2: A 12 year-old female from Northern Italy with ferritin 467 ng/ml, transferrin saturation 87-95%, and moderate hepatic iron overload. Material and methods. Direct sequencing of hemochromatosis genes (HFE-TfR2-HJV-HAMP-FPN-1) was performed in the children and siblings. Results. In case 1, we detected heterozygosity for a novel HJV mutation (g.3659_3660insG), which was inherited together with the beta thalassemia trait from the father, who (as well as the mother) had normal iron parameters. In case 2, we detected another novel HJV mutation (g.2297delC) in heterozygosity, which was inherited from the mother, affected by mild iron deficiency. The father had normal iron stores. Both mutations are frameshifts determining premature stop codons. No other disease causing variant was detected. Conclusion. Although beta-thalassemia trait was a possible cofactor of iron overload in case 1, iron overload cannot be explained by simple heterozygosity for HJV mutations in both cases. Other genetic factors should be investigated, and further studies are needed to understand genotype-phenotype correlations in JH

    The TM6SF2 E167K genetic variant induces lipid biosynthesis and reduces apolipoprotein B secretion in human hepatic 3D spheroids

    There is a high unmet need for developing treatments for nonalcoholic fatty liver disease (NAFLD), for which there are no approved drugs today. Here, we used a human in vitro disease model to understand mechanisms linked to genetic risk variants associated with NAFLD. The model is based on 3D spheroids from primary human hepatocytes from five different donors. Across these donors, we observed highly reproducible differences in the extent of steatosis induction, demonstrating that inter-donor variability is reflected in the in vitro model. Importantly, our data indicates that the genetic variant TM6SF2 E167K, previously associated with increased risk for NAFLD, induces increased hepatocyte fat content by reducing APOB particle secretion. Finally, differences in gene expression pathways involved in cholesterol, fatty acid and glucose metabolism between wild type and TM6SF2 E167K mutation carriers (N = 125) were confirmed in the in vitro model. Our data suggest that the 3D in vitro spheroids can be used to investigate the mechanisms underlying the association of human genetic variants associated with NAFLD. This model may also be suitable to discover new treatments against NAFLD

    La adquisición y reproducción en alta definición de señales acústicas implica cumplimentar diversos requisitos en función de las características de las señales y del sistema interviniente. La fidelidad del proceso de adquisición/reproducción de sonido, la latencia, la inmunidad al ruido y la diafonía son algunos de los factores que definen la calidad de un sistema de adquisición/reproducción de señales acústicas. Por lo general, en aplicaciones de metrología acústica, estos sistemas son de elevado costo y uso privativo. En este trabajo se presenta la comparación de la medición de latencia de dos controladores de audio que operan un sistema para aplicaciones de metrología acústica de bajo costo, evidenciando diferencias significativas entre ambos para dos condiciones de ensayo. Dicho sistema está conformado por computador personal, una tarjeta de adquisición/reproducción de sonido estándar y programas de cómputo numérico. El trabajo se presenta en el marco del proyecto de diseño y desarrollo de un sistema automático de medición de dispositivos acústicos, el cual permite caracterizar dispositivos acústicos activos y pasivos.Publicado en: Mecánica Computacional vol.     Survival in MS: a randomized cohort study 21 years after the start of the pivotal IFNB-1b trial

    OBJECTIVE: To examine the effects of interferon beta (IFNβ)-1b on all-cause mortality over 21 years in the cohort of 372 patients who participated in the pivotal randomized clinical trial (RCT), retaining (in the analysis) the original randomized treatment-assignments. METHODS: For this randomized long-term cohort study, the primary outcome, defined before data collection, was the comparison of all-cause mortality between the IFNβ-1b 250 μg and placebo groups from the time of randomization through the entire 21-year follow-up interval (intention-to-treat, log-rank test for Kaplan-Meier survival curves). All other survival outcomes were secondary. RESULTS: After a median of 21.1 years from RCT enrollment, 98.4%(366 of 372) of patients were identified, and, of these, 81 deaths were recorded (22.1% [81 of 366]). Patients originally randomly assigned to IFNβ-1b 250 μg showed a significant reduction in all-cause mortality over the 21-year period compared with placebo (p = 0.0173), with a hazard ratio of 0.532 (95% confidence interval 0.314-0.902). The hazard rate of death at long-term follow-up by Kaplan-Meier estimates was reduced by 46.8% among IFNβ-1b 250 μg-treated patients (46.0% among IFNβ-1b 50 μg-treated patients) compared with placebo. Baseline variables did not influence the observed treatment effect. CONCLUSIONS: There was a significant survival advantage in this cohort of patients receiving early IFNβ-1b treatment at either dose compared with placebo. Near-complete ascertainment, together with confirmatory findings from both active treatment groups, strengthens the evidence for an IFNβ-1b benefit on all-cause mortality. CLASSIFICATION OF EVIDENCE: This study provides Class III evidence that early treatment with IFNβ-1b is associated with prolonged survival in initially treatment-naive patients with relapsing-remitting multiple sclerosis
